To be considered for enrollment at O.M.S., children must be three years old (as of October 1st), fully potty trained, responsive to verbal direction, and no longer exploring orally.

Parents who decide to register a child will need to provide these items before your child will be allowed to begin school:

*	Tuition, registration, and supply fee.
*	Signed contract.
*	Completed application for admission.
*	Completed health history.
*	Health record, signed by your Dr.
*	Copy of child's birth certificate.
